A physician, hospital discharge planner, county case worker, client or family determines a need for home care services.

We provide an over the phone consultation.

The process of partnering you with a proper caregiver starts by listening to your needs. This is a no obligation consultation to answer your questions and for us to gather information that would be related to your care such as diagnosis, care needs, what caregiver qualities are important to you, family support, schedule, form of funding, etc.

 

We recruit and pre-screen qualified candidates.

Once we understand your needs, we recruit for your specific cares. We identify caregivers who are qualified to deliver the help you need, then carefully select the ones who are best suited to assist you with your long term daily needs.

We perform reference and background checks.

Our caregivers are certified through either the Minnesota Department of Human Services or Minnesota Department of Health, and must pass our strict screening and background checks before we will recommend them for your consideration.

We arrange an in-home meeting between the potential caregiver and yourselves.

This is a perfect opportunity for both to get to know each other and ask each other questions. We understand the importance of this relationship and want to be sure you are satisfied with your future caregiver.

We hire, Provide orientation, train and schedule your caregiver.

After selecting your caregiver, it is important that your caregiver attends our agency orientation covering their roles, responsibilities, policies, procedures and of course, hands-on training by our RN.

Our RN or Social Worker will arrange an in-home intake visit to develop a Care Plan specific to your needs.

A Care Plan is an integral part of your care. Our RNs and Social Workers will review and make changes as your care needs change.

We provide ongoing RN/Social Worker supervision and training.
